New Health Insurance Marketplace Coverage Options<\/a> – view this notice for information on coverage options available in the new Health Insurance Marketplace created under the Accountable Care Act.<\/p>\n

Annual Required Notice on Premium Assistance under Medicaid and the Children’s Health Insurance Program (CHIP)<\/a>.\u00a0 If you or your children are eligible for Medicaid or CHIP and you are eligible for coverage from ÀÖ²¥´«Ã½, your State may have a premium assistance program that can help pay for coverage.<\/p>\n

Medicare Part D – Notices of Creditable Coverage <\/a>– If you or a dependent are over age 65 and ending your coverage under the ÀÖ²¥´«Ã½ Medical Plan you will want to print out the applicable Notice of Creditable Coverage for each year you were covered under the ÀÖ²¥´«Ã½ Medical Plan after 65.\u00a0 These notices will prevent you from having to pay a premium for Medicare Part D Coverage.<\/p>\n

Notice of a Dependents COBRA Qualifying Event <\/a>– Send this form to Human Resources to inform us that one or more\u00a0of your dependents covered under the ÀÖ²¥´«Ã½ Medical or Dental Plans is losing coverage.<\/p>\n","protected":false},"excerpt":{"rendered":"
